Community Care Network 

Community Care Network (CCN) is a non-profit organization that provides

free health care services to uninsured, underinsured or otherwise underserved

populations living in rural or high risk urban areas.

Ø      Our goal is:

s         To improve access to care for underserved populations

s         To improve health outcomes

s         To reduce health care disparities among racial/ethnic groups and between rural/urban populations

Ø      We use our expertise and resources to address cardio-metabolic risk factors that disproportionately affect underserved populations

s         Cardio-metabolic risk factors include high blood pressure, high cholesterol, diabetes, obesity and tobacco use.

Ø      We currently have mobile clinics in Montgomery, Lowndes, and Wilcox Counties in Alabama. Additional clinics are planned for counties in the Black Belt region, South Rural and North Rural Alabama.

   In August 2003, CCN purchased a Mobile Medical Unit, which greatly enhanced our Medical Outreach capacity.  This Mobile unit, appropriately named the Care-A-Van, helps to overcome the lack of transportation that many underserved people face and enables services and treatment, which require private examination rooms and adequate laboratory facilities.  A second Mobile Unit acquired in 2007, is equipped with X-ray capabilities and a wheelchair lift.
